IMPROVING GLYCEMIC CONTROL IN HOSPITALIZED PATIENTS:
Strategies to Enhance Inpatient Care and Facilitate Outpatient Transitions

PURPOSE STATEMENT

This program will examine the importance of glycemic control in hospitalized patients, and discuss strategies for achieving and maintaining glycemic control in the hospital setting, as well as upon discharge.

LEARNING OBJECTIVES

Upon completion of this activity, participants should be able to:

  • Discuss the impact of uncontrolled glycemia on hospital patients and health outcomes
  • Explain the rationale for early and aggressive management of hyperglycemia in hospitalized patients with diabetes
  • Develop effective protocols to maintain tight glycemic control in hospital patients using insulin
  • Implement strategies to successfully sustain glycemic control in patients after hospital discharge

TARGET AUDIENCE

This enduring CME/CE series is intended for primary healthcare professionals, including physicians, physician assistants, nurse practitioners, nurses, and pharmacists, as well as for other specialists and healthcare providers who are interested in the management of patients with diabetes.
